Practice Information

Appointments

We see patients from 9:00 a.m. to 5:00 p.m. Monday - Friday. Appointments can be made by calling (801)268-4141 in Salt Lake and at (435)882-6448 in Tooele. If you need to be seen immediately, every effort is made to schedule you an acceptable time.

First Visit

On your first visit please bring your insurance cards and referral forms when needed or you can now download the Adobe Acrobat patient forms that can be printed and filled out to save you time when you come for your visit.

Please arrive ten minutes early to fill out general and medical history forms.

Cancellations

Because of time constraints we would appreciate at least 24 hour prior notification when unable to make a scheduled appointments. There may be a charge assessed for missed appointments.

Delays

Your time is valuable. Although we try to avoid delays, emergencies sometimes occur. If our doctors are delayed, our staff will inform you of the situation. If you feel you have been waiting too long, please discuss with the receptionist.

Cosmetic Consultations

There will be a charge for all cosmetic consultations and this charge would be applied to your surgery cost if scheduled.

After Hours Care

Within our medical group, an ENT physician is available twenty-four hours a day, seven days a week. You may reach this doctor by phoning (801)268-4141 in Salt Lake and at (435)882-6448 in Tooele, or you may proceed to an emergency room. The physician on duty will contact the on-call ENT physician from our group.

Financial Information

Your insurance company may require pre-authorization for certain procedures. Since the insurance industry is changing so rapidly, we would recommend that you verify with your insurance company if our group is listed as participating physicians. As a service to potential surgical patients, all participating insurances are verified and pre- authorized by our qualified office staff. Please help us by providing your insurance company identification card.

Payments and Copayments

Payments and co-payments, payable by cash, personal checks, Visa, Discover, American Express, or Master Card are required at the time of service. Charges not covered by your insurance company are your responsibility. Payment in full is expected within sixty days. All accounts exceeding 60 days will be charged an interest of 1 .5 percent per month(18% per annum). Returned checks are assessed a $20.00 fee.

Telephone Calls

Our office staff has been trained to answer most questions. If the question is one that only our physicians can answer our staff will consult with them and respond as as soon as possible. Generally we return calls during lunch and at the end of regular business hours. Urgent calls are given priority. If you believe that a prescription may be required, please have the phone number of your pharmacy available.

Prescription Refills

Many medications have indicated refills , which can be obtained from your pharmacy without calling our office. Because many diseases have similar symptoms, please understand that we may require an office visit before any medication is provided. If you have not been seen in the clinic for 6 months, we are not comfortable with the over-the-phone diagnosis. After office hours we do not have access to your medical records. Please advise us in as to your history, allergies to medications, etc., and remember to have your pharmacy phone number available.

Test Results

Any cultures done require 48 hours to obtain the final results. Blood work and x-ray results vary depending on where the tests were done. Any questions, please contact our nursing staff.
